1- A commercial register that includes the activity from the Authority's list of economic activities.
2- A title deed or lease agreement.
3- Appointment of a center director and a clinical studies follow-up officer.

Copy of the Title Deed or Lease Agreement
Proof that the establishment is operating in the country of origin and has been conducting the same activity for at least three years (for foreign entities). The document must be duly attested.
Detailed description of the center’s role in monitoring clinical studies
Copy of the Foreign Investment License (for foreign entities)
Copy of the National ID / Residence Permit (Iqama) of the Clinical Studies Officer
Copy of the CV, qualifications, and experience certificates of the Clinical Studies Officer (must be attested by the Embassy of the Kingdom of Saudi Arabia for foreign applicants)
List of the center’s Standard Operating Procedures (SOPs)
